To meet our goal of creating peace-of-mind in aged healthcare, we created the ‘Five Cs Model’ which guides everything we do:

Comprehensive. We take a holistic view of our patients’ mental and physical health, bringing together multiple specialities via the coordination of our GPs.

Connected. Communication is so important. We apply an open attitude and the latest technology to keep patients, families and homes well informed.

Convenient. We consult patients where they live and at a time that’s right. We aim to support the involvement of family members and are easily contactable.

Continuous. A regular visit from a familiar face can be positive for residents’ mental wellbeing. We emphasise the same doctor visiting the same facility.

Compassionate. Our doctors are experienced in dealing with the unique needs of aged care residents with sensitivity, thoroughness and proactivity.


The Tomorrow patient experience.

To become a patient of Tomorrow, nursing home residents and their family members can contact us directly, or we may be introduced to you by facility management.

Once you decide to be treated by our team, we’ll arrange for a handover of your medical history from the facility and any of your past GPs. During our initial consultations, we’ll talk to you about your medical situation and build up a relationship by understanding your interests and goals. It’s important for us that we get to know you personally and don’t hurry you along.

As we assess your health, we’ll perform physical exams and observations in a way that minimises any discomfort. Where appropriate, we’ll issue tests to gather more information about your condition.

We’re not here just for one initial consultation though. Tomorrow provides ongoing care to manage your health during an entire stay at a nursing home. We’ll schedule regular visits to check up on you, have chats about how you’re feeling, review medications and perform examinations where required. Our medical decisions will be based on the latest research and treatments.

Occasionally, we’ll draw on the expertise of specialist colleagues such as geriatricians, psychologists and physiotherapists. We want to manage your health from all angles.

One of the most disruptive and stressful things to occur, especially with end-of-life care, is a hospitalisation, so we’ll do everything within our power to keep you in the home where you’re comfortable (except of course where hospital care is necessary).

Periodically, we’ll schedule times to sit down and have multidisciplinary team meetings with care-givers and invite you as our patient as well as your family members to discuss the progress of your care.

We want you to love our communication, and we think this is what sets us apart. Our team will be available to take questions from facilities and family members as it’s important that they have the opportunity to be involved with your treatment. We use simple, jargon-free language to be as clear as possible, ultimately to keep you feeling control.
